Form Instructions
1. Fill-out fillable PDF forms completely.
2. Print and/or Save to computer.
3. Sign Forms. Please make sure forms are signed by principal of company.
4. Scan completed forms and save computer and send as attachment by email, or print and send by USPS.

Purchase orders that did not request a engraving proof and have been received by Bay Shore Woodworks, Inc. cannot be upgraded, changed, altered or canceled; no exceptions. Purchase orders that did request an engraving proof and were returned as "Approved" also cannot be upgraded, changed, altered or canceled; no exceptions.
